Export File

In the Export Options below, first select the FOA, the grantee(s), the instrument type and the date range for which you are exporting data (e.g. Adult Questionnaire, Youth Questionnaire, Individual Dosage, Group Dosage). Then click ‘Download’ to download your file and save it on your computer. Please note that the file will not include records that have been entered in the past 10 minutes.
